These terms explain how you may use the Phoenix PT Rehab website, the educational notes published on it, and the forms that run in your browser. Please read them with the privacy policy. If you do not agree, please do not use the site. You can still email care@phoenixptrehab.com with a question.

1. Who these terms cover

These terms are an agreement between you and Phoenix PT Rehab for use of this public website. They cover browsing, reading, searching, sharing links, and completing forms. They do not, by themselves, create a clinician–patient relationship. A care relationship begins only when a clinician has accepted you for assessment or treatment through the clinic’s ordinary process, not when a web form shows a confirmation message.

2. This is not emergency care

The website is for general information and for non-urgent requests. It is not monitored as an emergency line. Do not use a form to report chest pain, trouble breathing, sudden weakness, a seizure, a head injury, uncontrolled bleeding, thoughts of harming yourself, or any symptom that feels dangerous. Contact local emergency services or the nearest emergency department. Delay caused by waiting for an email reply can be harmful. We will not treat a website message as notice of an emergency.

3. Educational content is not personal medical advice

Articles about strength, posture, joints, balance, first visits, and surgery describe common rehabilitation ideas. They are written for a general reader. They do not know your imaging, your medicines, your surgical precautions, or your other conditions. A suggestion that is reasonable for one person can be wrong for another. Do not start, stop, or change an exercise program, a brace, or a medicine because of a page on this site. Use the notes to prepare better questions for a qualified clinician who can examine you.

Outcomes differ. We do not promise that pain will stop, that strength will return by a date, or that you will resume a sport or a job. Tissue, practice, sleep, other illnesses, and the details of an operation all change the arc. Any example on the site is a teaching illustration, not a report of a guaranteed result and not a testimonial you should treat as typical.

4. Clinicians named on the site

Profiles describe the focus of clinicians who practice with Phoenix PT Rehab, such as orthopedic, sports, neurologic, spine, pediatric, post-surgical, pain, cardiac, and mobility care. A profile is an introduction. It is not an offer that a named person is available at a particular hour, and it is not a statement of a regulatory license you should rely on without confirmation during the intake process. Services may be delivered by the clinician you request or by another qualified member of the team if scheduling, scope, or clinical judgment requires it.

5. Appointments, plans, and prices

The appointment form lets you select a clinician, describe a concern, and propose a date and time. Showing the confirmation message means the form was completed in your browser. It does not mean a time has been reserved on a clinic calendar, that a clinician has reviewed your history, or that a fee has been charged. A visit is scheduled only when the clinic confirms it with you, typically by email.

Pricing plans named Restore, Recover, and Renew are published so you can compare rhythms of care. They describe the kind of time and follow-up a plan is built around. They are not insurance benefits, and they are not a quote for surgery, imaging, equipment, or care delivered by other clinics. Prices can change. The figure that applies to you is the one confirmed before you begin a plan. If your needs are smaller or larger than the plan, we expect that conversation to happen before more visits are booked, not after a surprise.

You are responsible for arriving prepared, sharing changes in symptoms, and following precautions you have been given by the operating clinician or another treating professional. If you cannot attend, tell the clinic as early as you can so the time can be offered to someone else.

6. Forms on this website

Contact, appointment, subscribe, unsubscribe, sign-in, reply, and footer email forms check that fields are complete and sensibly formatted. They then show a thank-you message. As explained in the privacy policy, these forms do not send your entries to a server operated by this website. Do not type information you would only share inside a confidential clinical record. For a direct message, use the published email address. If a future version of the site begins to transmit forms, the privacy policy will be updated before that change, and these terms will be updated to match.

You agree that information you choose to type will be accurate to the best of your knowledge and that you have the right to provide it. Do not submit another person’s health details unless you are their parent, guardian, or otherwise authorized, and even then prefer a direct clinical channel over a public form.

7. Sign-in

The sign-in window asks for an email and a password so the page can confirm the fields are valid. It does not create a stored account on this website and does not unlock a private record room. Do not reuse a password you use for banking, email, or medical portals. Because the password is not transmitted by this site, it also cannot be used to authenticate you to us. Real identity checks, when a care relationship exists, happen through the clinic’s confirmed process.

8. Acceptable use

You may use the site for lawful, personal, and non-commercial purposes: reading, sharing links, and preparing for care. You may not misuse it. Without limitation, you agree not to:

• Attempt to probe, scan, or break the site, or to bypass any access control.
• Upload or transmit malicious code, or interfere with other people’s use.
• Scrape the site in a way that degrades it, or copy the design and text to present them as your own clinic.
• Use the forms to send threats, harassment, or unlawful content.
• Impersonate a clinician, a patient, or Phoenix PT Rehab.
• Rely on the site to market a product, collect addresses, or give the impression that we endorse you.

We may refuse, pause, or end access to the public site if use is abusive, unsafe, or unlawful. Ending access to a website does not, by itself, end a clinical relationship that was formed separately. Clinical endings follow professional duties, not only these terms.

9. Intellectual property

The Phoenix name and mark, the layout, the text, and the photographs made for this site are owned by Phoenix PT Rehab or used with permission. You may share links and quote a short passage with credit and a link back. You may not copy full articles, clinician portraits, or the logo into another commercial site, advertisement, or training product without written permission. Trademarks of other services, including social networks used for sharing, belong to their owners. A share button is not a partnership.

10. Privacy

Our privacy policy is part of these terms. It explains the limits of a static website, the sensitivity of health information, email, and the subscribe and unsubscribe choices. If these terms and the privacy policy ever conflict on a privacy question, the privacy policy governs that question.

11. Third-party services

Pages may load typefaces from a font host and may link to social networks so you can share a page. Those services have their own terms and privacy practices. We do not control them. Opening Twitter, Facebook, or Youtube leaves this site. Your use of those services is between you and them.

12. Disclaimer of warranties

The site is provided as available and as a general resource. To the fullest extent allowed by applicable law, we disclaim warranties of merchantability, fitness for a particular purpose, quiet enjoyment, and non-infringement, and we do not warrant that pages will be error-free, that a form confirmation equals a booked visit, or that educational notes apply to you. Some laws do not allow certain disclaimers. In those cases, the disclaimers apply only as far as the law allows. Nothing in these terms limits a duty that a clinician cannot legally limit once a real care relationship exists, and nothing here limits liability for harm that the law does not allow a clinic to exclude.

13. Limitation of liability

To the fullest extent allowed by applicable law, Phoenix PT Rehab and its clinicians, employees, and contractors are not liable for indirect, incidental, special, consequential, or punitive damages, or for lost profits or data, arising from use of the website, inability to use it, or reliance on its general articles. If we are found liable for a website-related claim that the law allows to proceed, liability is limited to the amount you paid us specifically for website access in the three months before the claim, or, if you paid nothing for website access, to zero. Clinical care that is actually delivered is governed by professional standards and by the agreement for that care, not by this website cap.

14. Your responsibilities

You are responsible for the device, browser, and connection you use, and for keeping your own email account secure. You are responsible for decisions you make from general reading. You agree to indemnify and hold harmless Phoenix PT Rehab from claims, losses, and reasonable legal costs arising out of your misuse of the site, your unlawful content, or your violation of these terms, except to the extent a claim is caused by our own willful misconduct. This indemnity does not shift responsibility for our professional negligence where the law says that responsibility stays with us.

15. Changes, suspension, and entire agreement

We may update the site, these terms, prices, hours, and clinician panels. The “last updated” date will change when these terms change in a material way. Continued use after an update means you accept the new terms for website use. We may suspend pages for maintenance or when content needs correction. These terms, together with the privacy policy, are the entire agreement about the public website. They do not replace consent forms or care agreements you sign for treatment.

16. Law and disputes

These terms are meant to be read under the laws that apply where you live and where the clinic is permitted to operate, without any requirement to name a single courthouse in this public page. If a dispute about the website arises, please email us first and describe the problem. Many concerns are misunderstandings about a form that never left the browser. If a dispute cannot be resolved informally, it should be brought in a forum that has proper authority over the parties. Nothing in this section keeps you from a regulator or from seeking urgent help.
